Tracking Software
This easy-to-use software helps Elementary School Milk Program coordinators keep track of the milk drinking habits of students registered on the program. It makes planning and ordering milk simple and easy!

Class List
Milk Coordinators can download a PDF version of the class list form here (PDF 35KB) to record the names of the students drinking milk at your school and to help with the ordering process.

Pre-payment form
Prepayment is the #1 tool to keep the program EASY FOR YOU and to keep students drinking milk.
Adopting a pre-payment system at your school will ensure the success of the program. Download the PDF parent pre-payment form here (PDF 47KB). The form indicates options for parents to order milk for their child in easy pre-paid amounts so the worry about their kids losing money at school is no longer a concern.

Parental Consent form
Another key to a successful program is encouraging full parental consent and involvement in the program.
Download the PDF version of the parent consent letter here (PDF 4.1MB) to inform parents about the program and to gain consent for their child's participation in the Elementary School Milk Program. You can use the letter as a template; photocopy it and send it home with each student at your school.

Cooler Bags
Your school will receive free cooler bags as part of the program. If you need more to keep milk cold from fridge to classroom, these attractive bags are available to order download the order form (pdf 680KB)

Punch Cards
To be used in conjunction with the pre-payment system, download and photocopy the milk punch card included here (PDF 908KB). Punch cards are used to collect payment for milk and to distribute the student incentives. Each time the student receives milk, the milk "Moonitor" hole-punches the card and when the card is full, the student can receive a milk reminder!
